Information gathered for this investigation determined a caregiver in the toddler room would push chairs in with their foot while children were sitting; resulting in several children falling off the chair. Same toddler caregiver forcibly sat a child on a chair. Same caregiver in the toddler room pulled a child's hair when removing a toy. The child had recently received surgery on their head and head was sensitive.

All classrooms had dust inside the toy bins and shelves. The infant classroom had two cribs with hairs on crib sheets. The headboard on one of the cribs had a mucus stain on the plexiglass. The infant classroom had two highchairs with stains on the safety straps. Classroom 102 D had a basket of arts and crafts on top of the play kitchen that poses a risk if the basket falls.

The infant caregiver pre-filled the daily infant report. It was 8 AM and the daily reports for all infants stated today's date and information on the infants day:"circle time, daily rountine, story telling, singing songs, child seemed happy and child slept well".

Classroom 102 D had four aluminum cookie cutters that were sharp.There were three empty aluminum cans and five lids that had rough edges. There was also a roll of masking tape in one of the toy bins. All these items were not age appropriate. Note. All items were removed by Child Care Regulation inspector.

The caregiver did not notice she was following the infants logs from last month. Infant caregiver searched for the current feeding logs that were found behind several documents on a tray by the hand wash sink.
