An assistant caregiver did not communicate with the lead caregiver or management that a child had fallen on the ground and hurt his/her shoulder during outdoor activities. The parents informed management the next day that the child received a fractured arm.

The caregiver was observed redirecting a child aggressively by grabbing and lifting the child by the wrist from the carpet to the trash can to throw away a tissue then escorted the child to a chair in a corner of the classroom to sit for timeout.

This standard was tasked over for this investigation and found to be deficient. A child was left unattended in his for approximately 14 minutes while his classmates were outside of the classroom. The child was found unattended in the class by his parent. The operation and director has implemented additional training and monitoring for the caregiver that was left iin charge. Observation of proper supervision was observed during the inspection time as well.

An operation allowed a person to be present at the operation before receiving notice from the CBCU regarding the person's eligibility. The person was removed from the classroom during the inspection. An administrative penalty in the amount of 100% of the daily maximum. per HRC 42.078(a-2) will be recommended as a result of the citation.
