There was a small hole in the flooring in the older 2 year old room exposing the concrete and accessible to children and that area was sharp to the touch. This was corrected when the area was covered. There was a sharp bent paneling out on the 12-18month old playground that was accessible to children. This was corrected by placing foam over it with tape. A broken toy was found on the infant playground that was accessible to children and sharp to the touch. This was corrected when the riding toy was removed.

Based on the video footage observed on the day of the incident, it showed a caregiver walking the children inside of the gym and when she gets inside, she realizes she was missing a child. The video shows her leaving the group of children in the gym unattended for less than a minute to go back to the classroom for the child that was left behind.
