It was determined throughout the course of the HHS Investigation, that multiple caregivers reported incidents to their administration per Region 14 policy, but they failed to report this to Child Care Regulation as a professional mandated reporter. Furthermore, Region 14 had advised all personnel that they must report concerns with the administration, contradicting the reporting mandate for CCR. Region 14 staff failed to also report the standard violations to CCR.
