The first incident Licensing Inspector was observed at the front desk when the Toddler caregiver was observed in the hallway. The second incident occurred at 7:56AM when the Toddler caregiver left 15 toddlers with Licensing Inspector in a classroom while retrieving breakfast waffles from the kitchen area.

While reviewing records, it was observed that the trainings provided were conducted by the Director, in which the operation had received an administrative penalty on 07/15/2021. The operation also received an administrative penalty on 11/19/2021. The operation Director provided 20-24 hours of training to their staff.
