This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff, it was determined that caregivers and administrative staff were aware of a broken cover for a hose connection box on the playground; however, the caregivers did not have an increased level of supervision for the box and as a result a child received a minor injury from the hose connection box.

During the observation inspection at the facility two infants were observed asleep in restrictive devices and one was asleep on a boppy in a sleep sacks. The infants were not removed immediately from the devices and placed in their cribs but were said to had fallen asleep after they were fed. The babies were eventually removed from the devices or awaken after approximately 3 minutes after the caregivers were instructed to do so.
