    Based on information obtained it was found that the building at the operation has not been maintained to be free from hazards. On the playground an area of the surfacing has been ripped open, with parts of the surfacing being raised and creating a tripping hazard. In the same area where the surfacing is ripped, there is a large dip in the ground that is hard to see unless you are on it. This creates a tripping hazard for the children. An area in the infant classroom has laminate tile that has raised and broken edges.
