During the course of a DFPS investigation, it was determined that multiple caregivers failed to demonstrate good judgement in the execution of their duties. The caregivers did not follow the operation's established protocol for accounting for children following a field trip. They failed to do a count of children, did not conduct a name to face count, and failed to conduct a full walkthrough of the bus which resulted in a child being left on a bus alone for approximately 15 minutes.
