A caregiver in the infant room did not wash hands after diapering a child and did not sanitize the diaper changing table. Furthermore, the caregiver did not was the infant's hands after diapering. Thsi was corrected at inspection when the caregiver washed their hands, washed the infants hands and sanitized the changing table.

3 outlets were observed to not have protective covers. The outdoor area had a green electrical cord in reach of children in care. The toddler room had diaper cream that stated "keep out of reach in children" accessible to children in care on top of the toilet. This was corrected at inspection when plug covers were placed on the outlets. The electrical cord was removed from the outdoor area and the diaper cream was placed out of reach of children.

The playground was observed to be littered with trash. The wooden fence surrounding the playground was observed to have screws protruding through the metal frame. A large gap was observed where the wooden fence meets the chain link fence. This gap is large enough for a child to fit through and/or become entrapped. An opening, large enough for a child?s head to fit through, was observed at the bottom of the wooden fence. The main hallway of the center was observed to be cluttered with large boxes filled with supplies, common household items such as a vacuum cleaner and carpet cleaner which was obstructing the hallway making evacuation difficult in the event of an emergency. Cleaning supplies and chemicals were observed to be within the reach of the children in multiple rooms. Unopened fly strips were observed on top of the microwave next to food items. Refrigerator observed to be unsanitary with dried food particles and spillage. Sink in school age room was clogged with standing water. Several classrooms were observed to be missing safety outlet covers.
