The water fountain in the outdoor play area is non working and has cords hanging form the unit. The outside electrical outlet was not covered properly. A window facing the outdoor play area was broken and cracked. A small play structure requiring surfacing was outside of the surfacing zone. Missing swings were observed exposing loose chain. Broken swings with heavy metal latches were observed laying on a ledge in the outdoor play area, in reach of children.
