Driver confirmed she has been transporting children in this van since it was purchased. Another van's Child Safety Alarm system was not in operation. Person in charge indicated the van was taken to get serviced and the alarm was disconnected. Person in charge confirmed the van has been used to transport children.

A six-month-old infant was observed sleeping in his crib on his stomach. Infant caregivers confirmed infant was placed on his stomach because infant allegedly is able to roll over. During this inspection infant was placed on his back but did not roll over. Photo was taken and uploaded to digital storage.
