A caregiver failed to adequately supervise a group of children during a class restroom break as one child wandered away from the restroom and back to the classroom unsupervised and without the caregiver's knowledge. In addition, based on information received, on multiple occasions children have been observed eating food that has fallen on the floor and eating food out of other children's plates and drinking out of other children's cups.
