A background check had not been submitted for this operation and was inactivated at the previous operation. The director submitted the background check request in the parent portal during the inspection. The caregiver will not return to the operation until an eligibility determination is received from the CBCU.

A large hole in the classroom wall was observed during the inspection. It is located in the children's library area of the classroom and is accessible to children creating a safety risk.

Three vans being used to transport children were observed to not have a fire extinguisher. These vans are rentals that are being used temporarily. This deficiency was corrected during the inspection by moving fire extinguishers that were located on buses that are no longer in use.
