Based on the video, a child was acting out and as the child was being spoken to by the teacher about this behavior, the child was seen hitting the teacher several times on the back and arm. The caregiver continued to pick up papers off the floor that the child throwing on the floor. The child continued to hit the teacher. When the caregiver turned around towards the child, she tried to grab for the child?s arm that was hitting her but missed and that is when the child hit the teacher in the face, the teacher reacted by swinging her hand that was holding the papers toward the child and the papers hit the child in the face causing a slight red mark on her cheek from the papers only. There was no bruise or bump. NOTE: The operation was proactive by immediately terminating this employee.

An infant saucer was observed in the room to be dirty with no child in it. A bookshelf in a room was observed to have dirt inside of it. There were several plastic containers in two rooms that were cracked and sharp to the touch for children. These were corrected during the inspection.

The caregiver was heard redirecting children from across the room, using negative terms such as "No, stop that, don't do that." The caregiver also threatened children with no outside time. The caregiver then grabbed a child by their cheek and told the child, "you don't say no to me."

A caregiver was observed yanking a child out of a book nook by their forearm and dragging them a short distance away. The caregiver then set the child down roughly on their bottom and then grabbed the child by their face while scolding them verbally. The operation immediately terminated the caregiver.

Upon review of the video, the caregiver was observed cleaning the room (stacking chairs and putting toys and nap mats away) for over 5 minutes while the children were running around the room and climbing inside the book nook.
