During the walk through of the operation the walls were observed to be written on, the wall trimmings were cracked with chipped paint,. The toys were broken on the playgrounds presenting risk of cuts, scrapes and entrapment, the playground had trash accessible to children in two seperate areas, due to recent rain prior to my arrival there were bodies of water in buckets, toys and structures, two silver structures were observed to be broken and placed in the center of the usage zones and the outdoor sink was filled with surfacing gravel rocks. The bathrooms within the classrooms had urine and feces requiring staff to walk around the center and flush all toilets, the bathrooms also had toilet paper on floors, potty trainings seats on the floors, a thick brown muddy substance cracked along the drainage hole in the toddler classroom, exposed toilet paper handles due to the covering on the san jamer holder being broken presenting risk of entanglement. The handwashing sinks were covered in brown stains. The window frames had broken window screens and the inside of the frames were filled with dust and dirt. The toys and equipment in the classrooms for learning were covered in crayon and colorful markings and the floors in the classrooms had small random items resembling trash on the floors presenting choking hazards.
