The remaining three personnel files were missing various items to include date of hire, affidavit of employment, signed acknowledgment of policy and procedures, proof of request for background check or eligibility letter, photo ID/driver's license, and proof of completion for orientation.

During the unannounced monitoring inspection, a 4 year old child in care was observed walking to the bathroom unsupervised. This was immediatly corrected when the caregiver exited the classroom to supervise the child in the bathroom.

During the walk-through inspection, one outlet cover was uncovered for the 3-4 year old class. Boxes were also piled high presenting a tipping hazard where the children in care nap. The bottom of the wall in the bathroom that children in care utilize needs to be fixed. The outlet cover was placed into the outlet placing the operation back into compliance. The boxes were moved to a storage closet, placing the operation back into compliance.
