In the outdoor area there are four slides, three slides were without loose-fill. One slide had loose-fill that is not the required 6 inches of depth due to mesh being exposed which can cause a trip hazard for children. Three of the four slides were removed from the outdoor play area.

In the pre-k classroom, there was medication exposed next to the medication bottle on the teacher's table which was accessible to children. The teacher removed the medication and was placed out of reach of children.

There were multiple hazards observed that presented risk to children: *The under lining around the playground border was exposed posing a tripping hazard. * There were sharp metal wires accessible on the fence posing an injury hazard if children were too close to the fence. *There were wood logs deteriorating on the playground that could cause insect infestation to the playground area. *There was a gap in the wire fence wide enough to fit the child's head and extremities. *There was a gap at the bottom of the wood fence wide enough to fit the feet of children, to become a tripping or entanglement hazard and expose the grounds to environmental hazards.
