The provider had recently identified hazards in the backyard play space but has not repaired or removed the hazards. The hazards included damaged wood fencing and a board with nails sticking up laying in the yard near the playground equipment. Deteriorating cardboard boxes with various items were also located in the backyard near the gate where the children pass through. The board with the nails was removed from the yard during the inspection.
