A caregiver was allowed to be alone with a group of children when the conditions of their background check stated that they cannot be alone with a child or group of children. This was corrected during the inspection when a 2nd caregiver without conditions came into the classroom.

During the inspection I observed children using a working restroom toilet that did not have a tank lid on it allowing the children access to the open tank. This was corrected during the inspection when staff replaced the tank lid.

During the inspection 2 children were observed placed in a restrictive device for disciplinary purposes. This was corrected during the inspection when the caregiver removed the 2 children from the restrictive devices.
