Through review of video and interviews with children and staff, it was determined a caregiver was not properly supervising the children in care who were using a large basketball game that had two basketball hoops and folding legs. The caregiver was not close to the basketball game and did not intervene to prevent the children from crowding around and leaning on the game which caused the legs to unfold and the equipment to fall. The top of the game hit a child, which caused minor injuries to the child.
