When observing the outdoor play area, due to the weather the playscape was wet and the children could have slipped and harmed themeselves. The hose used outside was not wrapped up and was accessible for children to trip and fall causing harm. The excess weed blocker used for the outdoor grounds was exposed and accessible to children. This was CORRECTED when the director wrapped the hose around the hose holder, cut the excess weed blocker, and the playground playscape was dried off.

Based on the information obtained, a child was able to run out of the front door of the building during a classroom bathroom break which the bathroom is in the front of the building. A staff member that was up front with the class during this break was able to run out the door behind her. The child only got a few steps from the front door before the staff was able to retrieve the child back into the daycare safely.
