During the inspection the following was observed: a child was observed walking from the restroom alone back outside without adult supervision, the toddler caregiver was observed walking out the classroom into the hall way while children were inside the classroom napping without adult supervision, and a toddler caregiver was observed to walk outside the classroom into the kitchen area while children were still in the classroom without adult supervision.

The diaper changing table located in the toddler classroom was observed to be dirty underneath the bottom surface with small white particles.

The carpet located in the toddler classroom was observed to have dark stains through out the carpet, food (bread) was observed on the carpet, the window was observed to have dust and stains. The ceiling fan and ceiling in the toddler room was observed to be dirty with dust, and cobwebs. The toys located on the playground were observed to be covered in dirt.

During the inspection it was found the toddler classroom was changed to the storage area without receiving prior notification from regulation
