During my arrival I saw a 5 year old child walked into the center from a transportation van without a caregiver into the center lobby. There was no one in the lobby. There were many times school age children were seen in the hallways without caregivers in the hallways with them.

The two slides located on the playground have 4 bolts sticking out of pipes that support the platform of the top of the slide. Each of the bolts stick out approximately 1/2 inch. The bolts are 5 feet off the ground and could catch a child's clothing. A child could also run into these bolts sticking out causing injury.
