Based on the interview with the parent, the Doctor's note submitted dated 1/13/2023 and the photos provided as proof stating the incident that happened on 1/11/2023 (screenshot). The parent stated during the interview that she always checks the child's body before she drops at the daycare on 1/11/2023, she knew the child does not have any bruises that day. But after the parent picked up the child in the afternoon, parent found the bruises and took a picture. The operation failed to file an incident report and did not share to the parent. During the inspection, the operation was asked of any other incident report aside from the 3 dates provided which the parent also provided the same copies. This standard was found to be deficient.
