A bathroom that is counted in the operations capacity was being used as storage. I spoke to the director about keeping the restroom usable for the children as it is part of their capacity. The restroom was cleared out. A toddler playground was not being used due to maintanence that was needed on the playground. There was fencing around the AC that was cracked and the toys were knocked over, and the cover for the sanbox was in the sanbox bunched up which could cause a tripping hazard.

Conditions that were placed on a caregivers background check was not being followed. This was corrected when the caregiver was placed in a different room with another caregiver without conditions. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

It was found during the investigation that a caregiver did not use good judgment when leaving children outside without a caregiver to take children to the restroom.

During the investigation it was found that a caregiver was leaving children unattended outside while going inside to help children go to the bathroom. It was also found that a child was wondering around the kitchen area without their caregiver. Another caregiver was present in the kitchen but did not acknowledge the child who did not have a caregiver. The child then went down the hall. It was a full minute before the caregiver came into the kitchen area with the class and noticed the child.
