Building grounds observed indoor area had visible wall and ceiling deterioration. Walls had visible chip marks and bottom trimming on the wall was coming apart wit nails observed to be sticking out. Ceiling had 4 tiles with water marks on ceiling, air vent had accumulation of dirt and dust. Restroom trashcan observed with an uncovered lid and soiled toilet paper inside. Caregiver specified restroom is used when the other is occupied, CORRECTED at inspection when trashcan was cleaned out and lid was placed in trashcan. Outdoor play structure had accumulation of dirt and cobwebs on and inside structures.
