- Critical
Supervision was not in compliance when a child was left alone unattended inside the classroom when the classroom transitioned outside to the playground.
source: Texas HHSC CCR verify - Critical
Based on information gathered in the investigation it was determined neither caregiver of a child left unattended on the playground noticed the child was missing when they got to the playground, when they left the playground, and when they returned to the classroom. They did not realize the child was missing from the group until they were alerted by the director. This substantiates that the caregivers did not know which children (or how many) they were responsible for. Information gathered indicated transition protocols were not followed.
