During the duration of the inspection, caregivers were heard from another room loudly telling children to be quite and watch tv. At one point during the inspection, a caregiver was heard telling a child to be quiet then continued by stating" you talk, talk talk,talk talk too much." The caregiver "shushed" the child then used their fingers to gesture to the child to zip their lips twice. Later, the caregiver was heard "shushing" children again putting their finger on their lip and telling children to be quiet.

15 child safety seats were observed to be expired on 3 of the vans used to transport children. 1 car seat with back, expired 2015. 14 booster seats between 2013 and 2022.

Shelter in place drills did not indicate they type of drill incidated. It is doucmented that a shelter drill was conducted each year 2025 but only 4 fire drills were conducted in 2025. Rat droppings were observed in the glove compartment on of the vehicles used by children. The contents of a the first aid kit were loose in the glove compartment and lying on the droppings.
