Riding toys were observed to have broken pieces, sharp chain link fence pieces protruded from the concrete right under the wooden fence. The operation transported two three-year-old children; the operation only had one forward facing child safety seat on the vehicle available for use. The operation staff did not know the weights or heights of any the fifteen children ages 4 through 7 years old that were transported; the operation could not determine if these children required more than a safety belt.

On 06/09/2025 the operation went on a field trip to a water park where there were twenty-four children with three staff members in the wading pool areas. The youngest children on the field trip were three years olds; the operation was over ratio by six children.
