    Based on the evidence obtained during the investigation, a caregiver was not actively supervising during the outside activities when sitting down engaged with their attention on their cell phone device. The caregiver did not have visual awareness of the on-going activities as two children were playing on the play equipment inappropriately that led to a serious injury.

    Various areas of the playground has tripping hazards. A broken concrete slab has a 1-2 inch raised edge near the dirt ground that presents a tripping hazard. A piece of exposed wood lifts up from the ground as well. Additionally a hole in the is big enough for a child to step in and has the potential to trip.

    One caregiver whom was hired on 12/10/21, and has been employed more than 90 days, has not completed CPR training or first aid training. Both trainings were required within 90 of employment which would have been due by 3/10/22.

    The outdoor space has three tricycles where the plastic seat is broken and cracked creating sharp edges and a pinch hazard. Additionally, the anchoring components of the climbing structure on the playground has exposed concrete pieces protruding out of the ground at both slide exits. There are also areas of the use zone where a combination of small and large hard rocks are protruding out of the loose fill.

    None of the nap mats had an indicated system to differentiate which side of the nap mats is the floor side and which side is the nap side. Additionally there were various mats torn with foam exposed which prevents them from being waterpoof.
