Based on a DFPS investigation, it was founded that the operation utilized a transportation van that did not have an electronic child safety alarm installed. The employee exited the vehicle without ensuring all children had exited.

Based on a DFPS investigation, an operation employee did not use good judgment when they exited the vehicle without ensuring all children had exited. As a result, a child was left unattended on the vehicle for over 30 minutes. It was also stated during the course of the investigation that children are sometimes allowed to turn off the electronic child safety alarm. Allowing a child to turn off the safety alarm does not demonstrate good judgment.
