    This standard was evaluated as part of the investigation and found deficient. A caregiver fed an infant less than an ounce of the wrong breast milk at feeding time.

    This standard was reviewed in relation to the self-reported allegations and found to be deficient. The caregivers did not properly supervise children in the Sanctuary resulting in a child's injury. Two caregivers did not demonstrate appropriate supervision when allowing children to stand and play on the adult sized chairs in the Sanctuary.
