    The operation did not have an updated fire inspection, the last fire inspection was conducted on 11-13-2024.

    A caregiver at the operation was found to have physically abused a child when two nail prints and a bruise were identified on the child's arm.

    In classrooms in the building the walls and floors were dirty. Paint is chipped off the wall. In the make believe boulevard classroom the ceiling tiles were coming down. On the playground the unitary surface had parts missing. A chest container on the playground did not have a lock on it for children to not have access to the filters in the container.
