On 04/08/2025, one child was left unattended in the Grow Fit classroom for approximately 3-5 minutes. The person in charge, did not notify the parent of the incident immediately, and/or during pick-up the day of the incident. The following day, 04/09/2025, the parent was not notified of the situation until pick-up, at approximately 6-6:30PM, more than 12 hours after the incident occurrence.

One child, 21 months old, was left unattended in the Grow Fit classroom for approximately 3-5 minutes. A staff retrieved the child when notified by another staff member. The child was returned to their primary classroom then after.

The Director did not address safety concerns brought to their attention by the childcare staff related to behavior, which resulted in numerous of incidents where children were injured. Staff was not provided additonal trainings related to working with children who experienced behavioral problems.

On 08/09/2024, one infant 12 months of age was grouped with school-age children, 4- 10 years of age. Based on the documentation observed the infant was present with the mixed group for approximately 20 minutes. A total of 15 children were signed in at one designated time.
