    The caregiver failed to ensure that all children were accounted for while transitioning the children out of the classroom located outside into the main building at the end of the day, resulting in a six-year-old being left unattended in the classroom for an undetermined amount of time.

The 4 year old playground had an unsecured gate that led out to a busy road. This was CORRECTED when the director secured the latch by placing a bungy cord on it. There was a jump rope found wrapped around the base of a play equipment on the 3 year old playground that presented an entanglement hazard. This was CORRECTED when the director cut the jump rope.
