This standard was evaluated and found to be out of compliance when a caregiver left a child in the bathroom, turned off the lights and closed the door for nine seconds on two occasions in the same day.

This standard was evaluated and found to be out of compliance. A child left the classroom via an open door and was on the playground alone for three minutes.

A door leading to the playground was left propped open without a door guard. While the caregivers tended to children's toileting needs and cleaning of the classroom, a child went out the door onto the playground.

This standard was evaluated and found to be out of compliance. An infant caregiver pulled, yanked, dragged, and pushed infants by the arm. The caregiver also carried infants by the arms across the room and threw one infant onto a padded play area.

746.2805(4) - Prohibited Punishments - Hitting with Hand or Instrument

This standard was evaluated and found to be out of compliance. An infant caregiver slapped infants' hands, arms, and mouth in response to infants getting into a cabinet, other prohibited areas, and when an infant tried to take off their own bow.

746.1203(4) - Responsibilities of Caregivers- Supervision of Children

This standard was evaluated and found to be out of compliance when a caregiver left a child out on the playground alone for 45 seconds.

746.3317(7) - Food Service and Preparation Requirement - No Foods That Present Choking Risk

The two and three year old children were served canned pear slices that were not cut up to prevent choking. This was corrected at inspection.

This standard was evaluated and found to be out of compliance. It was found that staff take shoes away from children when they do not listen to instruction or redirection.

This standard was evaluated and found to be out of compliance. It was found that caregivers have used cell phones and smart watches for personal use while supervising children.

Cleaning supplies were out on the counters easily accessible to children in two classrooms.

This standard was evaluated and found to be out of compliance. One staff was tending to a child in the bathroom while the second caregiver cleaned a table. The children were not supervised while playing by an open door that lead to the playground. A child went out the door alone.

746.3505(b) - Diapering Procedures - Wash Infant's Hands

In two separate classrooms, the caregivers did not wash the infant and toddler hands after diapering.

746.307(b)(1) - AP Parental Communication - Injury Requires Treatment by Health Care Professional or Hospitalization

This standard was evaluated and found to be out of compliance. The operation did not notify the parent right away when a child hit their head hard on concrete.
