The child safety alarm is not operating correctly according to manufacturer instructions. The button at the rear of the vehicle is not disabling the alarm as it is supposed to. When inquiring about daily procedures when transporting children, the operation has not been disabling the child safety alarm at the rear of the vehicle when using the child safety alarm.

Two caregivers were observed on the playground grabbing a child each by an arm and pulling the child across the outdoor activity space. The child was upset and then the child was held in a caregiver's lap and held in place for over10 minutes while they continued to be upset.

Some pickets are broken and present a pinch hazard. A piece of metal is protruding from the ground. A brick was placed over it at inspection. A dramatic play equipment that has tires has wired mesh exposed with sharp edge. The equipment also has a loose board. The barrier to the climbing equipment has cracks. And a bench that child use to sit on is missing boards and is unstable.
