    A former caregiver through the YMCA returned to serve as a substitute caregiver at this site, prior to their background check being re-ran by the organization. At the time of the inspection they were still marked as "Inactive" status. The employee was not counted in ratio at the time and promptly left until the background check is re-ran and returns as "eligible".

    Two children were able to get out of a classroom door and were unattended in an empty classroom by their selves for 1 minute and 11 seconds before they were found by staff.

    It was determined throughout the course of the DFPS Investigation that a caregiver used prohibited punishments with children in care. While reviewing camera video footage, a caregiver was observed to throw toys at, thus striking two children in care with the thrown toys, was observed to have grabbed and moved a child by their hair, and was observed aggressively push another child's head down. These three incidents were found to have occurred in a seven-minute timeframe.
