At time of inspection, a PreK caregiver stated they had seven children when they had eight.

Evidence gathered during the investigation indicated that the caregiver was supervising two groups of children. The caregiver was standing in the doorway between the playground and the hallway while also completing an incident report, which diverted the caregiver's attention resulting in two children engaging in inappropriate behavior while using the restroom.

A child was bitten by another child on the face while in care, resulting in significant swelling to the child?s eye and face. During the investigation, it was discovered the child who bit had history of biting prior to this incident. The caregiver saw this child walking toward the victim but did not intervene until after the biting had occurred; therefore, not ensuring the child?s safety. Additional bites were observed on the victim; however, there was not sufficient evidence to determine they were sustained while in care at the center.
