The operation allowed a child to go inside the building to the restroom and did not follow through with ensuring the child returned to the playground. The child was unsupervised inside the building for several minutes.

    While doing a walk though of the outdoor area, the play structure did not have sufficient loose fill in the use zone. The operation uses pebbles for loose fill and there was a small layer measuring less than and inch and the rest was compacted sand.
