During the course of the investigation, it was determined that toddler children had been frightened by a caregiver in a dinosaur costume who visited their classrooms after it was previously determined on other occasions that some of the children were afraid of the costume and the visits disrupted the classes and made some children cry. The costume was not associated with the curriculum.

Based on the information collected for this investigation, it was learned that the operation did not notify licensing when the staff failed to feed an infant their breast milk bottle on two separate occasions. It was discovered that one of the incidents, due to the lack of proper labeling of bottles, uncovered the infant caregiver served an infant the wrong breast milk bottle. Both caregivers in the classroom were aware of the situation and did not informed the director. Therefore, the operation did not notify licensing.

Information obtained for this investigation, revealed two infant caregivers were aware an infant was given the wrong breastmilk bottle and the parent was not informed of this mistake. Staff informed the parent of not being able to recognize which bottle belonged to the two infants with the same bottle, in which both had breastmilk.

The child was left unsupervised for approximately 3 minutes. Note: Caregivers conducted supervision training and were reprimanded for this incident.
