Based on the information gathered, the group transitioned back into the building and in the process a child walked into an empty classroom, while the rest group transitioned back into their classroom, leaving the child unsupervised.

The Toddler room had a loose cabinet knob and laminate that was peeling off of the counter. The infant room had a missing ceiling tile. There was a trash bag with old food by the fence on the open field side of the playground. A fence gate on the back playground next to the pool area has a gap over 5 inches from the ground to the bottom of the gate. A bathroom drain cover was not attached in the Prek classroom. An electrical control box for the door key pad off of the exit door to the back playground could be removed, exposing wires.
